Leads are converted, and no opportunity is created.
Conversion is the handoff from marketing to sales. Two things go wrong there, and neither shows up in a lead report.
Converted with no opportunity
The rep ticks "Don't create an opportunity". Sometimes that is right: the person joins a deal already open. Often it means pipeline that was never recorded.
Converted onto someone else's account
A lead converted onto an account owned by another rep is where disputes over deals and double-worked accounts start. It means the lead was routed without being matched to the account first.

In the second, count the rows where the two owners differ.
Have it checked every night instead.
